At The Arbor Extended Care, we provide a step-down phasing system. This enables our clients to effectively transition into independent living. Clients move through the phase system at their own pace, based on individual progress.

Each participant will progress through four treatment phases to guide the transition from high structure to increased independence:

Arbor Extended Care Program Phase 1 – Orientation

Sober living house residents cooking for themselves
This phase is designed to integrate the client into the program. They’ll begin to work the first steps in the 12-step process and participate in a home group

Goals

  • Participate in daily group activities
  • Complete steps 1-4
  • Obtain a sponsor
  • Establish a home group within a 12 step program
  • Successfully complete phase review session with recovery team

Phase 2 – Preparation

Clients continue to progress through the 12-step process, add new life skills and gain additional independence.

Goals

  • Complete Steps 5-8
  • Obtain a service commitment
  • Provide peer support to housemates
  • Begin working on discharge plan
  • Successfully complete phase review session with recovery team

Phase 3 – Integration

Participants in this phase of care are actively working steps 9-12, complete IOP, begin to mentor others and start to build their discharge plan. Phase 3 clients have more opportunities for independence, offsite interactions, and community integration.

Goals

  • Actively work steps 9-12
  • Begin mentoring peers through step work
  • Complete IOP
  • Begin Recovery Advocacy Services and Care Plan
  • Continue participating in service work at home group
  • Complete Discharge Plan
  • Successfully complete phase review session with recovery team
  • Actively seek employment, education, or volunteer service opportunities
  • Demonstrate recovery principles in all areas of one’s life

Section G – Phase 1

  1. Action Items
    • Actively participate in all daily scheduled activities, goals and group outings.
    • Timely submission of personal requests (sponsor and family visits, etc.).
    • Consistent self-initiative with responsibilities.
    • Acknowledge and respond appropriately to input from peers and staff.
    • Have an active relationship with a sponsor.
    • Complete Steps 1- 4 and schedule a date/time for Step 5.
    • Complete assignments on time.
    • Submit Phase Two application when areas listed above have been met successfully.
  1. Structure
    • After the first week, onsite family visitation for up to 3 hours.
    • Access to personal laptop, phone, and tablet from 7:00am – 10:30pm. With exception of during groups, outings, meetings, IOP and meals.
    • Weekly walk up to 2 hours on Saturday or Sunday, accompanied by two buddies (phase 2 or phase 3).
    • Three weekly 30-minute store runs accompanied by two buddies (phase 2 or phase 3) on  Monday, Wednesday and Friday.
    • Sponsor may come to house for step work.
    • Daily phone call(s) to sponsor.
    • Maximum personal spending of $65/week, plus $15 to be reserved for group outings.  Financial resource availability and timing is the responsibility of each resident.

Section H – Phase 2

  1. Action Items
    • Maintain all Phase One Action Items
    • Complete Steps 5-8
    • Demonstrate willingness and consistency to process life happening’s and recovery issues in healthy ways.
    • Be a leader within the community (be proactive in supporting and helping others).
    • Sign up and attend service commitments.
    • Begin searching for employment or approved equivalent.
    • Complete IOP.
    • Meet with Recovery Advocate to develop Care Plan.
    • Submit Phase Three application when areas listed above have been met successfully.
  • Structure
    • Two off-site passes and two on-site visits, with family per month for up to 3 hours.
    • Access to personal laptop, phone, and tablet from 7:00am – 10:30pm. With exception of during groups, IOP and meals.
    • Weekly walk up to 2 hours on Saturday or Sunday
    • Three weekly 30-minute store runs on Monday, Wednesday and Friday.
    • Attend two outside meetings per week for up to two hours with another phase 2/3 or sponsor
    • Attend job interviews and/or educational sessions, medical/dental appointments, outside therapist appointments, other appointments/activities as scheduled.
    • When offsite, and not with extended care group for meetings/outings, residents may have personal cell phone and gift cards. Residents will be provided with a staff member phone number to text hourly or call in case of an emergency.
    • Sunday evening submit weekly schedule to recovery manager to include: Offsite passes to meetings, service commitments, 12-step work, and social activities.

Section I – Phase 3

  1. Action Items
    • Maintain Phase One and Phase Two Action Items.
    • Actively working steps 9-12.
    • Actively mentoring peers.
    • Meeting weekly with Recovery Advocate and working on Care Plan
    • Begin participating in a service commitment at a designated home group
    • Maintain a weekly schedule.
    • Actively searching for employment.
  2. Structure
    • Three, 5-hour off-site passes and one, 8-hour off site pass with family per month.
    • Access to smartphone, laptop, and tablet from 7:00am-10:30pm.
    • From 10:00am – 5:00pm (M-F) resident may leave property to work on employment interviews, recovery, education, or other treatment-plan goals.
    • Work or volunteer up to 40 hours weekly.
    • Attend up to three outside meetings per week for up to three hours in duration
    • Eligible for one, 24-hour pass monthly as approved by Recovery Manager & Advocate
    • Graduate/senior housing as available
