A significant goal of the Arbor Extended Care Program is to ensure all participants leave the program with a solid base with the 12-Step Process and the life skills required to lead a healthy and successful life.  There are four extended care treatment phases to the program, and each individual is expected to progress through all of them before graduating.

Arbor Extended Care Program Phase 1 – OrientationSober living house residents cooking for themselves

Expectations

  • Must abide by all aspects of resident code of conduct
  • Participate in all daily group activities.
  • Complete Steps 1-3 and begin Step 4
  • Have and Identified Sponsor
  • Have an Identified Home Group
  • Be accepting of contracts given by staff and complete all assignments given
  • Submit Phase Two application
  • Successfully complete phase review session with recovery team

Privileges

  • Eligible to participate in group outings
  • After first two weeks, eligible for monthly onsite family visit
  • One ten minute phone call weekly

Phase 2 – Preparation

Expectations

  • Maintain all Phase One expectations
  • Complete Steps 4-8
  • Demonstrate willingness to process issues in healthy ways
  • Accept and respond appropriately to input from staff and peers
  • Be a leader within the community (be proactive in supporting and helping others)
  • Begin working on discharge plan
  • Work diligently in all areas of recovery (unity, service, and recovery)
  • Sign up for service commitments
  • Submit Phase Three application
  • Successfully complete phase review session with recovery team

Privileges

  • All phase one privileges
  • Twenty minutes phone time – Maximum two Calls
  • Access to personal laptop or tablet PC during study hall
  • Five dollars walking around money
  • Eligible for one weekly 30-minute walk
  • Sponsor may come to house up to twice weekly for step work
  • Two monthly onsite family visits

Phase 3 – Integration

Expectations

  • Maintain all Phase One & Two expectations
  • Actively working step 9
  • Begin mentoring peers through Step work
  • Complete IOP
  • Begin participating in service work at your home group
  • Complete Discharge Plan
  • Submit Graduate Phase application (including one week schedule)
  • Successfully complete phase review session with recovery team
  • Complete a Check-In sheet after each solo outing.

Privileges

  • Two monthly family visits – one of which can be offsite for 3 hours
  • Access to personal laptop or tablet PC during study hall and approved times during weekends
  • Attend Aftercare Weekly
  • Eligible for two weekly 30 minute walks
  • Eligible to attend up to two meetings solo per week
  • May meet offsite with sponsor
  • May attend Group Conscience at their home group
  • Eligible to go offsite for job interviews and education
  • Supervised internet and phone access for employment purposes

Phase 4 – Graduate Phase

Expectations

  • Maintain all Phase One, Two, & Three expectations
  • Actively working Steps 10-12
  • Demonstrate recovery principles in all areas of one’s life
  • Follow through with your personal weekly schedule

Privileges

  • Eligible for 24-hour pass
  • Access to personal cellphone while off property
  • Eligible to leave property independently from 12 pm-5 pm during the week to work on employment, recovery, education, or other treatment-plan goals.
  • Eligible to be independent from program schedule for one day during the weekend.

Note: All privileges are contingent on staff approval and availability. Phases can be revoked if current phase expectations are not being met.

Graduate Phase clients are still required to attend all scheduled activities and meetings when they are present at the house.
